If you are already enrolled in the Plan and wish to enrol your spouse and/or dependants, select 'Enrol Your Family Now' from the Change-of-Coverage menu to use the online family enrolment system. Online enrolments are secure, convenient, ensure quick processing and allow you to receive an instantaneous confirmation of your Change-of-Coverage request.

Enrolment Form - Fall 2010

January Enrolments
Eligible students beginning their studies in January who wish to enrol themselves (and their spouse/dependants) for coverage from Jan. 1 - Aug. 31, 2011 should visit this page at the beginning of January to download the January Enrolment Form.

Spring Term Enrolments
New students beginning their studies in the Spring Term who wish to enrol themselves and their spouse/dependants for coverage from May 1 - Aug. 31, 2011 should visit this page at the beginning of May to download the Spring Term Enrolment Form.
