Self Enrolment

Enrolment Forms are available in the Download Centre during the Change-of-Coverage Period.

Eligibility
New students enrolled in the Winter Term are not automatically covered by the SU Health & Dental Plan. They may enrol themselves in the Plan by completing an enrolment form and paying the appropriate fee during the Change-of Coverage Period.

As Plan members, students who complete a self enrolment may also choose to enrol their spouse and/or dependants by completing the couple/family enrolment process outlined in the Couple & Family Enrolments section.

Only SU members may enrol themselves and their spouse/dependants in the SU Health & Dental Plan. If you’re not sure of your coverage status, please contact studentcare.net/works.

Enrolments can’t be processed after the deadline. Coverage is only for the current policy year.

Deadlines
All enrolments must be completed during the Change-of-Coverage Period, between Jan. 6 - 18, 2010 for coverage from Jan. 1 - Aug. 31, 2010.

Students who are on leave and who are interested in being covered by the Plan must contact studentcare.net/works.
