Enrolment FormIf you are already enrolled in the Plan and wish to enrol your spouse and/or dependants, select 'Enrol Your Family Now' from the Change-of-Coverage menu to use the online family enrolment system. Online enrolments are secure, convenient, ensure quick processing and allow you to receive an instantaneous confirmation of your Change-of-Coverage request.If you aren't automatically covered, but are eligible to enrol yourself, you will not be recognized by the online system and cannot enrol online. You can still enrol yourself and your spouse/dependants by completing an Enrolment Form.You can also drop off your Enrolment Form at the Health & Dental Plan Office. January EnrolmentsStudents beginning their studies in January who are not automatically covered, but who are eligible and wish to enrol themselves (and their spouse/dependants) for coverage from Jan. 1 - Aug. 31, 2012, will not be recognized by the online system and should visit this page at the beginning of January to download the January Enrolment Form.
Enrolment Form - Winter 2012Spring Enrolments New full-time graduate students who were not on Academic Term in Fall 2011 or Winter 2012 and who wish to enrol themselves and/or their spouse/dependants for coverage from May 1 - Aug. 31, 2012 will not be recognized by the online system and should visit this page at the beginning of May to download the Spring Enrolment Form.
