The Plan covers the cost of new or replacement eyeglasses or contact lenses. The maximum allowed is $75 per 24 months (eligible 24 months after the full amount has been claimed).

Please note that when purchasing contact lenses online, you are required to submit a copy of your valid current prescription with your claim.
